All prices are in Canadian dollars. Add 5% Goods and Services Tax (GST).

Registration includes breakfast, lunch and two refreshment breaks for two days, attendance at two full days of sessions, the Wednesday welcome reception, and a copy of the conference program and CD upon publication.

To register please download and print the following registration form: SCM09 Symposium Registration PDF and mail or fax it with your payment to the PMAC National Office at the address below.

If you are a member of PMAC you can pay by Visa, MasterCard or American Express through PMAC’s secure online registration and MiraPay payment system at SCM09 Symposium Secure Online Registration & Payment. If you are not a member of PMAC, you can still use the online payment system, but you have to first obtain a login and password by sending a request to info@pmac.ca.

Cancellation Policy

Cancellations received up to September 30, 2009 will receive a full refund less $100 administration fee. No refunds or credits will be given for cancellations after this date. All refund requests must be made in writing to the PMAC National Office. All no-shows are charged the full amount.

PMAC reserves the right to make changes to the program without notification. If the program is not held for any reason, PMAC’s liability is limited to the conference registration fee.
